General Information

ALL tutors MUST complete fingerprinting clearances before they may work with students. Tutors MUST be fingerprinted in the state they were assigned to work with NOT the state they reside in.

Fingerprints completed previously for other organizations or companies cannot be used. Please carefully follow all instructions in order to avoid delays.

Initial Fingerprinting Assignment

Your initial fingerprinting assignment is located on your welcome email sent to your personal email address.  This email is titled “Subject: Congratulations! We want you to join Amplify!” and fingerprinting instructions and location are under Step 3. Amplify will only reimburse you for the fingerprints you have been requested to complete. Feel free to reach out to tutorhiring@amplify.com with any questions.

*Please note: As a virtual tutor you will often be working with students outside of the state you reside in.  You must be cleared in the state the students you are working with live.  This means that you may be submitting fingerprints to a state other than your own. For each state, we provide fingerprinting instructions for tutors who live IN that state and fingerprinting instructions for those tutors who live OUT of that state.  Please ensure you are following the correct set of directions to avoid delays.

Additional Tutoring Assignments

As new tutoring engagements and needs arise, you may be asked to complete additional fingerprinting clearances beyond your initial assignment.  If Amplify requests that you complete additional fingerprinting, you will reimbursed for those costs.

**Amplify will only reimburse TCW Tutors for tutoring assignments you have been requested to fingerprint for.

Ink Fingerprinting on Hard Cards

Many tutors who are completing fingerprinting out of state will need to make an ink fingerprinting appointment. Actual ink fingerprinting or digital scans that can be printed onto a hard card are both acceptable

Finding a Service Provider:

  • Check with your local police station or law enforcement agency to see if they offer this service.
  • Search “ink fingerprinting near me” in google or another search engine to find other service providers in your area.
  • Call ahead to find out if an appointment is needed or if they accept walk-ins.

Questions to ask prior to your appointment:

  1. What do you need to bring with you to the appointment?
  2. How much will it cost and what payment methods do they accept?
  3. Do you need to provide the hard card or do they have them available at the location?
  4. How many hard cards with fingerprints will they complete for you? (We recommend getting at least two cards in case the first one is rejected by the state)

Electronic Fingerprinting

  • Follow the instructions for your assigned state to locate an electronic fingerprinting location.
  • Call ahead to find out if an appointment is needed or if they accept walk-ins.

Documentation

As part of the registration process, you will be required to provide your demographic information, including but not limited to your full legal name, date and country of birth, nationality, as well as your biometric information. 

You will also need to provide the details of one of the following forms of IDs: 

  • State ID or Driver’s License 
  • Employment Authorization Card 
  • Permanent Residence Card 
  • U.S. Passport 

Please note that the aforementioned documents must be unexpired and must reflect the exact legal name as listed on the registration form.

**Please contact the agency prior to your fingerprinting appointment to ensure you have any other documentation that they may need to avoid delays.

Ink Fingerprinting

If you need to complete ink fingerprinting please call the agency that will be taking your fingerprints and ask if you need to supply your own hard card or if they provide the hard card for you. If you do need to provide your own hard card, an FD-258 hard fingerprint card can be purchased from many places including Amazon and Walmart.

Amplify will reimburse you for the cost of purchasing a fingerprint hard card. Please keep copies of all receipts.*

How to avoid fingerprinting delays

  1. Include all required documents.
  2. Use your legal name that matches your documentation.
  3. Make sure that all costs are paid for using the correct format required by the state and for the exact amount required. (For example, CA requires a $49.00 money order for out of state fingerprints.  This MUST be a money order for EXACTLY $49.00 or it will be returned).
  4. Double check addresses, codes, and names are correct.

Ink Fingerprinting Tips

  • When having your ink fingerprints taken, make sure that the prints are not smudged and are clear. Moisturize hands and hydrate 24 hours in advance for the best prints.
  • It is common for fingerprints to be rejected for poor quality, so we suggest obtaining two sets of fingerprints in case you need to resubmit down the line.
  • Fill out hard cards completely with accurate information

Reimbursement Procedures

Tutors hired through Kelly Staffing are eligible for a reimbursement of one hour or prep time.  Kelly Services does not reimburse for fingerprinting expenses.

Upon completion of the Fingerprint Submission Verification Form, Kelly Services will be notified that you have completed fingerprinting. Please reach out directly to them with questions related to reimbursement.

Tutors hired through TCWGlobal are eligible for reimbursement of fingerprinting expenses.

If your TOTAL COSTS will be above $150 (per state), you must have prior approval for reimbursement over that amount.  Please contact tutorhiring@amplify.com using the subject line: “Fingerprint Reimbursement Prior Approval”.

Please submit the following fingerprinting submission and reimbursement form once your Amplify email address has been set up by the IT department. You must submit a separate form for each state, if a receipt covers more than one state only include it one time in your expenses. For example, if you mailed both Ohio and Arkansas prints at the same time and have one USPS receipt you will only include it in your Ohio submission and will input $0.00 for Arkansas expenses.

You will need to upload all receipts (or other proof of payment ex. copy of check/money order).

There are several potential costs you may incur depending on your fingerprinting location:

  • Cost charged by Fingerprinting agency to complete the fingerprints

  • Cost to the state completing the clearance

Upon submission of fingerprints (completed your electronic fingerprinting appointment or mailed your hard card through USPS), please fill out this form to inform the Amplify tutoring team. 

This will allow us to keep track of your submission and clearance.

Upon submission of the google form you will receive a code.  Please hold onto this code as you will need it to provide proof of submission during your tutor training.

If you receive clearance results directly

Some states like Ohio or Delaware may send results directly to the tutor.  If you receive results directly, please fill out the Hardcopy Results Form to submit proof of results to the Amplify Tutoring Team.

Note: Please submit your results even if an issue is found, as exceptions can be made depending on the offense.

Arkansas

Instructions

IN-STATE CANDIDATES:

Step 1: Pay online with a debit or credit card . Read the instructions at https://www.ark.org/ade/licensure_cbc/index.php After submission of the form, print or save the receipt to take with you to your fingerprint facility and to submit to Amplify for cost reimbursement.

  • Choose the Arkansas Department of Education in the dropdown menu.
  • Reason Fingerprinted – Choose: EDN Non-Teacher Employment (Classified/Substitute)
  • Enter your PERSONAL email address to receive your receipt. Your Amplify email will block this receipt.

Step 2: Complete the ONLINE CONSENT FORM. Copy and paste this URL link: https://aels.ade.arkansas.gov/AELS/Consent/Consent1.aspx

  • Check the “Employment (Classified)” box.
  • Print the form when completed to take with you to the fingerprinting location.

Step 3: Visit a Live Scan (electronic) fingerprinting location. You will need to bring: 

  • A copy of the receipt with the transaction number (an electronic copy is acceptable)
  • A copy of the ONLINE CONSENT FORM (an electronic copy is acceptable)
  • Your government-issued photo ID

INK PRINTS: If you cannot find a Livescan location near you, you can obtain ink fingerprints at the Arkansas State Police headquarters or local law enforcement.

  • Bring the entire printed consent form along with the copy of the online receipt, which has a required transaction number.
  • Bring a completed DEPT. OF EDUCATION FINGERPRINT CARD (obtained by submitting your name and mailing address to ade.educatorlicensure@ade.arkansas.gov). The hard card will be mailed to you.
  • Bring your government-issued photo ID.

Step 4: Arkansas Child Maltreatment Central Registry Check

Please complete this form and follow the instructions. Select “You are a teacher, pre-service teacher, work for or attempting to work for a school in Arkansas” and then “Non-Licensed/Classified”. Fill out the Applicant Information Section (use personal NOT Amplify email) and Children Section.You will receive a copy of the form to your email. Download and save the form.

You will then be directed to complete a second form. (See the example screen shot for how to fill in the fields). You will upload the first form into the second form and complete payment.  Please save receipt for reimbursement.

OBTAIN RESULTS AND NOTIFY AMPLIFY:

After placing your background check materials in the mail  please fill out this form to confirm your submission of your fingerprint background check.

For assistance please call the licensure office at 501-682-4342 Monday through Friday between the hours of 7:30AM – 4:30PM.

Fill out the  Results Upload Form including an attached screenshot or PDF of your results.  This is how the Amplify Tutoring Team will know if the background check cleared or not.

REJECTIONS: 

If fingerprints are rejected by the FBI, the applicant will be asked to obtain inked fingerprints. The State Police will send the applicant a letter and the instructions in that letter should be followed explicitly. If the inked fingerprints are also rejected, the FBI will conduct a records check using the applicant’s name.  

Instructions

Step 1: Request a DEPT. OF EDUCATION FINGERPRINT CARD. Email the request to: ade.educatorlicensure@ade.arkansas.gov. Submit your name, mailing address and Indicate this will be for a non-certified teacher and is for employment purposes. The hard card will be mailed to you.

Filling in the Arkansas Department of Education Fingerprint Card:  The fingerprint card will come with the Reason Fingerprinted and ORI sections pre-filled. You will need to fill in the following sections of the card- name, signature of person fingerprinted, residence of person fingerprinted, Employer and Address: Amplify, 55 Washington St. Suite 800, Brooklyn, NY11201, Citizenship (ex. USA), Social Security Number, Sex, Race, Height, Weight, Eyes, Hair, Date of Birth, Place of Birth. The person completing the fingerprinting should fill in Date and Signature of Official Taking Fingerprints.

  • Your government-issued photo ID

*We recommend calling the fingerprint facility first to verify they have ink prints so you can utilize the card provided by the Arkansas Department of Education.

All tutors supporting California students are required to complete the California DOJ’s fingerprinting process before working with students. California residents can make an appointment with an approved Live Scan Operator to complete this process.

To do so, please complete the following steps:

  1. Print out Amplify’s Request for Live Scan Service document and the accompanying billing form located under the Printable Resources.  
  2. You do not need to complete any information on the billing form. Simply print the form and bring it with you to your appointment.
  3. Complete the “Applicant Information” section of the Request for Live Scan Service document beginning with the Last Name field and ending with the Applicant Signature and Date fields.
  4. If for any reason you are asked to submit a second set of prints please reach out to tutorhiring@amplify.com for further instructions.  
  5. You do not need to complete any information on the billing form. Simply print the form and include it with your other documents when completing the fingerprinting process.
  6. Locate the nearest approved Live Scan location using this website.  Please note that most locations require you to schedule an appointment in advance.
  7. Bring the following items to your appointment:
    • Printed and completed Request for Live Scan Service document and the Billing Form.
    • Valid photo identification. Could include State ID or Driver’s License, Permanent Residence Card, or U.S. Passport. Expired identification will not be accepted.
    • Acceptable form of payment. Amplify has a billing code (listed on the Request for Live Scan Service document) that will be accepted by some Live Scan locations to cover all or a portion of the fees associated with your background check. However, some locations only accept cashier’s checks and money orders. Review the acceptable forms of payment listed for each Live Scan location when scheduling your appointment and be sure to bring one of those forms of payment in case, along with the billing form you printed from the Printable Resources section.*
  8. Upon completion of the appointment please fill out this form to inform the Amplify tutoring team of your submission. 

*If you incur any fees when registering or obtaining your background check, please keep copies of all receipts and Amplify will reimburse you for the fees. Please see instructions for reimbursement in the General Information section of this website.

  2. You do not need to complete any information on the billing form. Simply print the form and include it with your other documents when completing the fingerprinting process.
  3. Complete the “Applicant Information” section of the Request for Live Scan Service document beginning with the Last Name field and ending with the Applicant Signature and Date fields. *Please note:  If you are out of state, you will not be completing Live Scan fingerprinting. Electronic live scan fingerprints completed out of state will not be processed by California.  However, you must still include the Request for Live Scan Service document in your packet because California will then process your hard card through live scan.
  4. Call a local agency that takes fingerprints or your local police station and ask if they do scans to hard-card printouts or if they do ink prints, in which you need to supply the hard card. Each agency is different. If you do need to provide your own hard card, an FD-258 hard fingerprint card can be purchased from many places including Amazon and Walmart. Amplify will reimburse you for the cost of purchasing a fingerprint hard card. Please keep copies of all receipts.*
  5. Have your fingerprints taken on the hard card. When having your fingerprints taken, make sure that the prints are not smudged and are clear.   6. Please see the sample hard card (under printable resources) and ensure the information is completed accordingly.  You will need to include the ORI code: AT571 and Employer Address: Amplify, 55 Washington St. Suite 800, Brooklyn, NY11201
  7. Mail your completed Request for Live Scan document, Billing Form, hard card and $49 money order made out to California Department of Justice, Prescan Unit (money orders only, no personal checks or cash. Note: Must be made exactly for the $49 or it will be returned.)  to:

Instructions

If your fingerprints are rejected, you will receive an email from our HR department which includes the California DOJ rejection notice.  This notice is very important as it includes your unique original ATI number.

In the instance your prints are rejected a second time due to low quality, the CA DOJ will run a manual check using your full name and you will not have to submit your prints a third time.

However, it is imperative you follow the instructions EXACTLY below or the DOJ will not identify your submission as a 2nd submission and will not run the manual check.

You will NOT have to submit another $49.00 as long as your LiveScan form and your card are labeled as a resubmission.

For In-State Applicants using LiveScan:

It is important that your Live Scan operator enters the second transaction as a resubmission, utilizing the ATI from your previous rejected transaction in the OATI field in order to link the resubmission to the initial rejection. This can be found in the email sent to you by our HR team which included a Rejection Notice attachment.

  1. Print out a copy of your rejection notice to bring with you to your appointment.
  2. Print out Amplify’s Request for Live Scan Service document and the accompanying billing form located under the Printable Resources.  
  3. You do not need to complete any information on the billing form. Simply print the form and bring it with you to your appointment.
  4. Complete the “Applicant Information” section of the Request for Live Scan Service document beginning with the Last Name field and ending with the Applicant Signature and Date fields. Under the “Your Number” section, be sure to include the OCA # (AT571) and where it says “If re-submission, list original ATI number:” list your original ATI (OATI) number from your rejection notice.
  5. Be sure the Live Scan operator enters the second transaction as a resubmission.

For Out-of-State Applicants using Hard Card Submissions:

If a fingerprint hard card is used for resubmission, the card must be an original FD 258, FBI Applicant Fingerprint Card (the downloaded version is not accepted). The fingerprint card for the resubmission must have the OATI number entered in the space between the “CLASS” and “REF” fields (the OATI would be the ATI number provided on the initial reject response). Please note that all information (DOB, App Type, etc.) in the resubmission must be the same as in the initial transaction. A resubmission cannot be used to make corrections or add a level of service and the OATI may only be used once. See the example of the sample hard card.

Follow the instructions below:

  1. Print the Rejection Notice PDF and take with you to the fingerprinting facility. This is a copy of the results we received either from the CA DOJ or FBI. Some fingerprinting sites do want to see proof of rejection before resubmission and will often not charge you to have fingerprints redone. The Rejection Notice PDF will include your OATI # (Original Automated Transaction Identifier #) which you will need to fill out in the resubmission section in the Request for Live Scan Service document.
  2. Print the Blank Request for Live Scan Service PDF and complete the “Applicant Information” section in the Request for Live Scan form. Under the “Your Number” section of the form, make sure to fill in the Resubmission Section with the OCA # (AT571) and your original ATI (OATI) #, which will be in the Rejection Notice PDF. (If you did not receive it, contact the tutoring team).
  3. *Please note that ALL information (Full name, DOB, App Type, etc.) in the resubmission must be exactly the same as the initial transaction.
  4. Print the billing form. You do not need to complete any information on the billing form. Simply print the form and include it with your other documents when completing the fingerprinting process.
  5. When having your fingerprints taken, make sure the prints are not smudged and are clear. Moisturize hands and hydrate 24 hours in advance for the best prints.
  6. You will not have to submit another $49.00. Mail your completed Request for Live Scan documentBilling Form, and hard card to

Once you enter the service code, you will be routed to a new page. Click on Schedule or Manage Appointment to continue. 

Follow the instructions and fill in all required fields. Hit Next>. To proceed to the following pages and fill in all required information: Essential Info>Additional Info>Citizenship>Personal Questions>Personal Info>Address>Documents. 

**Please use your PERSONAL EMAIL address when completing IndenToGo information**

You will NOT be using an Authorization/Coupon code as requested in the Personal Questions section.  You will be asked to provide payment method information when registering at the end of the process.

Results

Fingerprinting results will be sent to the school district and they will notify you of the results.  Once you obtain your results, please fill out this form, which will notify your tutoring team that you have been cleared.  Make sure to keep a copy of the letter for your records and you will be asked to upload this as proof.

Out of State or No IdentoGO Location Nearby

If you are unable to visit an IdentoGO location in-person, or if there are no physical locations near you, you have the option of submitting a fingerprint card by mail. Please note that the mail-in option takes 4-6 weeks to process.

To mail in your fingerprint card, start the registration process online at https://uenroll.identogo.com. Then proceed to type in the district service code 27RY1H

Once you enter the service code, you will be routed to a new page. Please click on the Submit Fingerprint Card By Mail option as shown below.

After submitting payment you will receive confirmation of your pre-enrollment and a printable summary. You must print, complete, sign, and mail the summary document along with your fingerprint card to the address listed on the provided document.

Before mailing in the form you must obtain an FD-258 fingerprint hard card and get a copy of your fingerprints from a criminal law enforcement agency. Blank hard cards can be purchased from many places including Amazon. Amplify will reimburse you for the cost of purchasing a fingerprint hard card.

When having your fingerprints taken, make sure that the prints are not smudged and are clear. Moisturize hands and hydrate 24 hours in advance for the best prints.

Fill out the fingerprint card with YOUR information and leave the rest blank. Make sure to include the printout from your registration which should have included a bar code and instructions, which will accompany the card.

Compile the printed registration form as well as your completed fingerprint card and mail them to the address listed on the registration form and below. Make sure prints are mailed in a large envelope and are not folded.

In order to begin in-person or virtual tutoring within Baltimore City Public Schools, you are required to complete and clear a fingerprinting background check through Baltimore City Public Schools’ fingerprinting service. To maintain safe social distancing and to ensure the safety of employees and customers, all fingerprint/background checks will operate by appointment only. Participants are required to wear a mask or facial covering when visiting the office for a fingerprint/background check.

The cost of the fingerprint/background check is covered by Amplify. Please provide the following code to the staff person assisting you at the time of your fingerprinting and you will not have to pay out of pocket. The code is BCPS-0020.

To schedule a background check appointment, please go to https://booknow.appointment-plus.com/b04ebxmq/ and select a date and time you are available to come in. When completing the online registration form, please enter “BCPS-0020” in the account number field. Please also include, “Prepaid via Amplify’s Bithgroup account” in the special instructions section. Baltimore City Public Schools asks that individuals make every effort to be present and on time for their scheduled fingerprint/background check appointment. Instructions

To be a tutor in PGCPS, all tutors must follow these 2 steps.

  1. Undergo a fingerprint check -Results must be received prior to engaging in tutoring activity. See the section on Fingerprinting below.
  2. Fill out the Background Disclosure Form and the Child Protective Services Form (needs notarized). See full instructions below.

*Note: While filling out any forms for fingerprinting, DO NOT use your Amplify email address, as you will not be able to receive emails from outside sources. Use your personal email only.

STEP 1: Get fingerprinted.

In-State Applicants. All in-state applicant fingerprints submitted to CJIS for the purpose of licensing and/or employment background checks must be done electronically via approved live scan fingerprinting units. The only time in-state ink-and-rolled ‘hard-card” applicant fingerprint cards will be accepted will be when there is a bona-fide medical reason a live scan transmission cannot be obtained. 

Please select a Maryland live scan location to schedule an appointment.  Fees: Fingerprinting and Child Protective Services(CPS) Clearance is $61, payable by personal check. You will need to make sure to bring one of the following forms of identification in order to be fingerprinted.  

  • MD driver’s license or driver’s license issued by another state
  • United States passport or unexpired foreign passport.
  • Certificate of United States citizenship or naturalization.
  • Alien registration card.
  • United States Military identification card.
  • Identification issued by state or local Government.

Out-of-State Applicants. You must request a fingerprint card by:

Writing the Criminal Justice Information System – Central Repository to: P.O. Box 32708, Pikesville, Maryland 21282-2708; 

or

Calling the Criminal Justice Information System – Central Repository in Baltimore City at 410-764-4501 or via the toll free number 1-888-795-0011.

Once you receive the card, you will need to take the fingerprint card to a local fingerprint vendor that can take the fingerprints and print them out electronically on the fingerprint card or contact your local police department or other vendor who will perform the ink and roll method . Please note that the appearance of the fingerprint card may be different from the one pictured below and may have different instructions than what you see here. Please follow only these instructions.

The most important step is to ensure the PGCPS authorization code (1300005711) is on the card. In the “Reason Fingerprinted” section the applicant should check “State and FBI”. In the “Criminal History Request Type” section the applicant should check “Child Care”.

Include a personal or cashier’s check for $30.00 (make sure it is the exact amount) made out to the “CJIS Central Repository”. 

Once mailed fill out the Fingerprinting Submission Verification Form and include the receipt from fingerprint vendor and the USPS Tracking # for the certified mail. This will notify the Amplify Tutoring Team your forms have been mailed and will help us track the results.

Upon receipt of the fingerprint card, the response from the Criminal Justice Information System – Central Repository can be expected within 10-15 days. Results will be sent to Prince George County Public Schools and they will notify Amplify of the results.

Fingerprint cards from other states and/or the Federal Bureau of Investigation cannot be accepted.

STEP 2: Complete Additional Forms.

Complete and send the Background Disclosure Form and the Child Protective Services Form (needs notarized) to the address below.  The Child Protective Services Form must be notarized (keep your receipt) and mailed to the address below. The Disclosure Form can be sent at the same time and does not need to be notarized. Please keep a COPY of these forms as you will need to upload them in the below form for tracking purposes. Mail these forms via certified mail (keep your receipt) prior to being fingerprinted to notify the district of whose results they will be waiting for.

Tutors will need to have both a BCI and FBI fingerprinting card completed at the selected organization, as well as a request for exemption form.  All three of the documents below can be found in the Printable Resources section. It is recommended that you print out the provided Ohio fingerprinting cards on cardstock to bring with you to the appointment.

Write 1AB002 in the Agency Code box. 

Where it asks “SEND BACKGROUND CHECK RESULTS TO” mark both boxes and write in ODE by Direct Copy and fill in with your mailing address.

The ‘Reason Fingerprinted’ field must be completed. Please check “ORC” and write 3319.291

Each fingerprint card must be completed in full with required information (i.e., social security number, date of birth, etc.) All information should be typed or printed legibly. If any of the information is incomplete, fingerprint cards will be returned unprocessed.

FILLING OUT THE FBI FINGERPRINT CARD

In the OCA (Agency Code) box, enter Agency Code 1AB002. In the ‘address result is to be sent to’ write and then circle this address: Department of Education

The ‘Reason Fingerprinted’ field must be completed. Please check “Other, please specify” and write 4UR619.

In the Employer and Address field fill in with DOE by Direct Copy and add your mailing address.

COSTS: OUT-OF-STATE
